Overview:

Under general supervision of a nurse manager accordance with agency policy, provides professional nursing care services with emphasis on service to patients requiring care related to public health programs; performs professional nursing in accordance with prescribed rules, regulations, professional practices, and special orders from physicians; may supervise licensed practical nurses and/or home health aides, and performs related work as assigned by nurse manager.

Job Description/Duties:

	Provides skilled nursing care and prescribed medical treatment to patients and demonstrates care to patients, families, practical nurses, and aides.
	Identifies, observes, reports and records symptoms and conditions of patients.

	Assists as a team member in the development of nursing care plans.
	Gives medically ordered immunizations, tuberculin tests, treatment and medication.
	Assists with medical examination, takes histories, and performs screening and recording activities at clinic and schools.
	May give directions and assignments to licensed practical nurses and/or home health aides.
	Helps explain agency services and policies to persons receiving service and to other persons with whom she works.
	Gives general information to the public about the local public health agency and its services.
	Attends staff meetings and participates in continuing educational programs.

Knowledge and Skills Qualification :

	Considerable knowledge of professional nursing theory and practice, with ability to apply nursing techniques. Knowledge of medicines and narcotics.
	Ability to assess nursing needs and implement nursing care plans.
	Ability to administer skillfully medically ordered medications and treatment.
	Ability to apply nursing techniques.
	Ability to follow oral and written directions.
	Ability to keep records and charts and to make reports.
	Ability to effectively deal with the public.
	Ability to supervise and train non-professional personnel.
	Ability to establish effective relations with physicians, patients and fellow workers.

Education and Experience Qualification:

Graduation from an accredited school of nursing and possession of current licensure as a registered nurse in Minnesota.

Under the supervision of a professional nurse, gives nursing care to patients at home and participates in clinic activities; is responsible for recognizing and reporting significant changes in the patient's progress to appropriate responsible persons; and performs related work as assigned by a professional nurse.

Job Description/Duties:

	Participates in routine patient care work, involving making beds, changing linens, lifting and moving patients, giving baths and rubs and feeding patients.
	Observes patients and reports unusual conditions or behavior to the professional nurse in charge.
	Assists physicians in treatments when needed.
	Takes vital signs (temperatures, pulse, respiration and blood pressure rates.)
	Writes records of care and treatment given and records changes in patient's condition.
	Administers prescribed medications and treatments under close supervision according to agency policies.
	Assists physicians in treatments when needed.
	Assists in physical examinations and other clinic activities.
	Replenishes medical supplies.
	Gives routine treatments within the scope of her training and ability.
	Gives patients baths and other personal care.
	Teaches appropriate self-care as directed by the professional nurse.

	Knowledge of daily hygienic care of patients.
	Knowledge of measures to prevent spreading of diseases.
	Knowledge of and ability to use sterilization equipment.
	Knowledge of methods of handling sterile materials and dressings.
	Knowledge of first-aid assistance to render in emergencies.
	Knowledge of practical nursing theory and practices.
	Ability to establish effective relationships with patients, fellow workers, and the public.
	Ability to interpret works to others, patients' families and staff co-workers.
	Ability to do accurate recording.

Type of Work:

Under the direction of nurses, performs personal health care services in cases not requiring the judgment of nurses; may perform home management or housekeeping services under the monitoring of a nurse or a Home Health Aide Coordinator; and performs related work as assigned.

Tasks Include:

	Performs bathing, toileting, feeding, and other personal care services for patients.
	Assists patients with prescribed exercises and activities.
	Assists patients with ambulation and with the use of special equipment.
	Assists patients with meal planning and preparation.
	Assists patients with basic housekeeping tasks.
	May assist in providing adult/child in homes in which the parents are physically or mentally disabled
	Performs clean dressing changes under the supervision of a nurse.
	Ensures that patient takes self-administered medication.
	Reports any changes in the patient's mental or physical condition to the nurse and shares with the nurse and/or Home Health Aide Coordinator his/her observations about the home situation.

Knowledge and Qualification Requirement but Will Train:

	Knowledge of personal health-care practices and principles.
	Knowledge of the principles of home management.
	Knowledge of child care methods.
	Knowledge of the elements of nutrition and meal planning.
	Knowledge of first aid skills including CPR and home safety.
	Knowledge of the aging process and accompanying behavior changes.
	Knowledge of the emotional problems accompanying illness.
	Knowledge of child maltreatment and vulnerable adult reporting laws.
	Ability to accept and adapt to varying life styles and home environments.
	Ability to follow oral and written directions.
	Ability to retain confidentiality of home conditions and situations.
	Ability to communicate effectively orally and in writing.
	Ability to establish effective working relationships with clients.

Minimum Qualifications Of Education And Experience:

Completion of Minnesota Department of Health approved vocational training for home health aide. Will train for the right candidate.

	Pass both criminal and driving history screening.
	Must be at least 18 yrs of age.
	Must attend 64 hours of special vehicle driving training (8 of 8 hours session). Fee will be reimbursed.
