Clients who meet the following criteria will be accepted for services:
Procedures: When the Preliminary information is completed and the referral information is confirmed, the file containing the supporting information will be given to an assigned nurse (RN/LPN) for intake/assessment scheduling. Such cases will be seen immediately by the most available nurse (RN/LPN) to facilitate any crisis or pro stabilization. Routine assessments, financial intakes, etc., may be completed subsequent to the initial contact at the direction of the case nurse (RN/LPN). Information of recipient rights may not be deferred. |

The nurse (RN/LPN) or the administrative assistant will schedule an appointment to conduct an intake, screening, and assessment interview. The interview shall be scheduled no later than six working days beyond the date of initial contact unless so requested by the client and recorded in the record.
